169-175 Russ: Worker at/above 15 feet. This plank is not a scaffold platform. There are no safety rails. Worker is not secured by a lanyard. Worker not wearing hard hat.

The Carpenters Union cares about safe working conditions on the job. Safety walks at Joe Cassidy Construction, Inc. job sites show what we believe to be a lack of regard for worker safety, including OSHA and Fire code violations.

Issues include:

» Lack of Fall Protection: We found an uncovered elevator shaft, missing handrails on staircases, and missing safety rails.

» Fire Hazards: Job site gates are locked, putting workers’ lives at risk in the event of fire. We also found a lack of fire extinguishers at the sites.

» Lack of Personal Protection Equipment: Hard-hats, safety goggles, and ear plugs are rarely available or used.

» Unsafe Lighting: Staircases and corridors lack proper lighting at many sites.

The photos below, taken on safety walks by the Carpenters Union, raise serious concerns about unsafe work practices and job site safety hazards.

29th & Dolores: 3rd floor windows: no safety rails for fall protection
2nd floor: large open penetration for lumber is unprotected
2nd floor: 2x6 from the structure to the scaffolding is unsafe and illegal
29th & Dolores: 7/25/03 Photos indicate safety rails installed when police arrived. Unsafe and illegal 2x6 from building to scaffolding is still being used.
7-25-03: Supervisor locking gate to keep the union out after a safety walk, danger to workers, no fire extinguisher on site
29th & Dolores: Wall at stairwell is not properly secured. Carpenters are framing on sub-floor next to unsecured wall.
29th & Dolores: Workstation should be kept clean and free of debris, trip hazards and fire hazards. No fire extinguisher on site.
29th & Dolores: Cal OSHA requires toe boards around all open penetrations to prevent objects from falling to floor below
29th & Dolores: Ladders should always be opened up when used. Platform is too narrow, illegal and lacks fall protection.
29th & Dolores: Stacking material against scaffolding reduces its structural integrity
Pneumatic nail guns should never be used without eye protection. Workers are not using PPE, hardhats, safety goggles, ear plugs.
169-175 Russ: Scaffold lacks rolling wheels with safety locks.
29th & Dolores: No handrail, safety rails at stairwells, photo taken on third floor
29th & Dolores: Handrails installed, not proper height, no toe boards for falling objects
